To help ensure that we are covering only those dependents in 2026 who meet our eligibility requirements, and that our plans remain cost-effective for everyone, you will need to provide documentation for dependents enrolled in Entegris plans.
We will conduct a full dependent verification audit in early 2027 – more details will be shared closer to its start. We recommend using Annual Enrollment to review the dependents you cover, and confirm each one is still eligible.
If you enrolled dependents without documentation prior to Open Enrollment in 2026, you will need to provide eligibility documentation as outlined in the table below. If you add new dependents during open enrollment and upload documentation then, no action will be needed for the audit.
