Medical Plan choices

Entegris offers three Cigna Medical Plan choices to keep you and your family healthy—two High Deductible Health Plans (HDHPs) with a Health Savings Account (HSA) and one Preferred Provider Option (PPO).

Each plan includes:

  • Annual deductibles — what you pay before the plan begins sharing costs
  • Out‑of‑pocket maximums — the most you’ll pay in a year for covered services
  • Prescription drug coverage through Express Scripts
  • Nationwide provider access, with lower costs in the Cigna network

Making the Most of Your Plan

No matter which plan you choose, keep these key tips in mind to get the best value from your medical coverage:

  • Use in‑network providers and pharmacies — You’ll always pay less when you stay within Cigna’s medical and pharmacy network.
  • Choose urgent care when appropriate — For non‑life‑threatening issues, urgent care typically costs significantly less than the ER.
  • Take advantage of free preventive care — In‑network preventive services are covered at 100% (no cost to you). This includes annual physicals, immunizations, screenings, and routine lab work designed to keep you healthy.

The way deductibles and out-of-pocket maximums are determined depends on the plan you choose. The table below shows the difference in types – read on for more explanation.

Deductible differences are:

  • Embedded deductible: Each person has their own smaller deductible “cap.” If one person hits their individual deductible, the plan starts paying coinsurance for that person even if your whole family hasn’t met the family deductible yet.
  • Non-embedded deductible: There’s only one big family bucket. The plan doesn’t start paying for anyone until your whole family’s combined expenses reach the family deductible.

Out-of-pocket maximum differences are:

  • Embedded out-of-pocket maximum: Each person has an individual out-of-pocket maximum within the family plan. If one person hits their personal cap, the plan starts covering 100% for that person, even if the family hasn’t hit the overall out-of-pocket maximum.
  • Non-embedded out-of-pocket maximum: Only when the family’s combined eligible spending reaches the family out-of-pocket maximum does the plan begin covering 100% of costs.

At the start
of the year

  • You pay the full cost of most medical and pharmacy expenses, except those with a copay, until you reach the deductible.
  • If you are enrolled in the HDHP Option 1 or 2, you can pay for these expenses from your HSA. If you are enrolled in the PPO plan, you can pay for these expenses with a Health Care FSA.

Once your meet
your deductible

  • Once your deductible is met, you and the plan share the cost of covered medical and pharmacy expenses.
  • The plan will pay a percentage of each eligible expense, and you will pay the rest.

Once you reach your
out-of-pocket maximum

  • Your deductible, copays and coinsurance count toward the out-of-pocket maximum.
  • Once you reach the out-of-pocket maximum, the plan pays 100% of the cost of covered medical and pharmacy expenses for the remainder of the year.
